Prefer Print & Fax?

1.	You can find the reseller application here and the use certificate here. The use certificate can be typed online by clicking on the lines that you would like to fill out.
2.	Once you have filled out both the reseller application and the use certificate, you will need a copy of your tax certificate.
3.	Fax all three completed forms to us at (828) 459-7341.
